Dear Sir:

 

Henry Jones is a retired Treatment Plant Operator II who worked at the O’Brien Water Reclamation Plant. On May 27, 2016, he reported he sustained an injury to his lower back while raking debris off screens and then using a pulley system to move a dumpster. He was seen at the industrial clinic on May 27, 2016. The doctor disabled the employee from work effective May 30, 2016. The employee was treated conservatively with medication and physical therapy, however, the employee failed to improve. Further diagnostic testing revealed a large herniated disc at L3-4. He had additional treatment consisting of several steroid injections and several radiofrequency ablations. The employee returned to work on August 12, 2016.

 

Mr. Jones was paid a total of 10.40 weeks in lost time benefits totaling $12,914.52.    

 

Mr. Jones filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $18,880.50, representing approximately 5% loss of use of the person as a whole.   This settlement will close out future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
