Dear Sir:
 
Request for Proposal 14-RFP-32 documents have been prepared for Third Party Claims Administrator Services, at the request of the Human Resources Department. It is anticipated that the service agreement(s) will be effective on April 15, 2015 or sooner.
 
The purpose of this contract is to obtain services to administer claims for the District in three categories: Category 1) Workers' Compensation claims and related services, with an option to also provide Category 2) Non-litigated Liability claims and related services; and Category 3) Claims Management Services for large or complex losses on an as-needed basis. Vendors may submit a proposal in all three categories. Alternately, Vendors may submit a proposal related to only Category 1) Workers' Compensation or for both Category 1) Workers' Compensation and Category 2) Non-litigated Liability.  Vendors may not submit a proposal for only Category 2) Non-litigated Liability.  Vendors may also submit a proposal for only Category 3) Claims Management Services.
 
The estimated cost for this RFP for Category 1 is $140,000.00 for 2015, $175,000.00 for 2016, $185,000.00 for 2017, and $50,000.00 for 2018. The estimated cost for Category 2 is $19,000.00 for 2015, $24,000.00 for 2016, $24,000.00 for 2017, and $5,000.00 for 2018. The estimated cost for Category 3 is $40,000.00 for 2015, $50,000.00 for 2016, $50,000.00 for 2017, and $10,000.00 for 2018.
 
A bid deposit is not required for this RFP.
 
Appendix A will be included in this RFP.  The type of work to be performed under this contract is within the professional services category for establishing Minority-owned Business Enterprises (MBE), Women-owned Business Enterprises (WBE) and/or Small Business Enterprises (SBE) goals.  The MBE, WBE, and/or SBE goals for this contract are:  20 percent MBE, 10 percent WBE, and 10 percent SBE.

Funds are being requested in 2015, 2016, 2017, and 2018 and will be contingent on the Board of Commissioners' approval of the District's budget for those years.
