Dear Sir:
Lawrence Deschamps is an Electrical Mechanic who works at the Egan Water Reclamation Plant. On January 9, 2013, he sustained an injury to his left hand. He was initially seen at the industrial clinic on January 10, 2013. The diagnosis was left thumb extensor pollicis longus tendon rupture. After going to a specialist, surgery was performed on January 18, 2013. The surgery performed was a left extensor indicis proprius tendon transfer to extensor pollicis lungus tendon. Following this surgery, Mr. Deschamps underwent a course of physical therapy. The employee was able to return to work on May 2, 2013.
Mr. Deschamps was paid a total of $16,325.88, which represents 14.8 weeks of lost time benefits.
Subject to the approval of the IWCC, this Pro Se case can now be settled for $36,518.18, representing a 25.0% loss of the left hand. This settlement will close out any future lost time benefits, as well as any future medical benefits, associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
