Dear Sir:
 
On June 13, 2012, Christine Del Giudice was working as a Senior Office Support Specialist at the Stickney Water Reclamation Plant when she sustained an injury to her right hand. On June 13, 2012, she went to the industrial clinic and was diagnosed with right thumb contusion and pain. She received conservative care before later undergoing surgery. The preoperative diagnosis was contusion of the right thumb and wrist with carpal tunnel syndrome, de Quervain tenosynovitis, and dorsal ganglion cyst.  On September 4, 2012, she underwent surgery for right carpal tunnel release, decompression of first extensor compartment, synovectomy of extensor pollicis brevis tendon, excision of dorsal ganglion cyst, and decompression of extensor carpi radialis brevis tendon. Following surgery, Ms. Del Giudice received a course of physical therapy. On September 27, 2012, she was able to return to work.
 
Ms. Del Giudice was paid a total $2,209.50, which represents 3 and 3/5 weeks of lost time benefits.
 
Subject to the approval of the IWCC, this Pro Se case can now be settled for $22,647.17, which represents 20.0% loss of the right hand. This settlement would close out future lost time benefits as well as any future medical benefits, associated with this injury.
 
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
