Dear Sir:
Jerry Culhane is an Operating Engineer I at the Stickney Water Reclamation Plant. On February 1, 2013, he sustained an injury to his right shoulder. He was initially seen at the industrial clinic on February 1, 2013. On May 24, 2013, an MRI of the right shoulder revealed further injury. On July 10, 2013, the employee underwent a procedure. On December 16, 2013, he was able to return to work full duty.
Mr. Culhane was paid a total of $32,314.04, which represents approximately 28.4 weeks of lost time benefits.
Mr. Culhane's attorney filed an Application of Adjustment of Claim with the Illinois Workers' Compensation Commission (IWCC). Subject to the approval of the IWCC, this case can now be settled for $45,068.79, representing approximately 25% loss of a right arm. This settlement will close out future lost time benefits, as well as any future medical benefits, associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
