Authority to settle the Workers’ Compensation Claim of Kirk Vopelak vs. MWRDGC, Claim W000867064, in the sum of $26,720.63, Account 901-30000-601090

Dear Sir:
Kirk Vopelak is an Architectural Ironworker at the Stickney Water Reclamation Plant. On March 18, 2013, he sustained an injury to his lower back and upper left leg. He was initially seen at the industrial clinic on March 21, 2013. The employee’s treatment consisted of physical therapy and injections.
Mr. Vopelak did not lose any time from work, but a total of $23,200.21 was paid in medical benefits.
Subject to the approval of the IWCC, this Pro-Se case can now be settled for $26,720.63, representing 7.5 % loss of person as a whole. This settlement will close out any future medical benefits, associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
