Dear Sir:

 

Henry Negoda is a Machinist at the Stickney Water Reclamation Plant. On April 20, 2010, he sustained a minor injury to his right fifth finger. There was no lost time involved. 

 

On July 20, 2010, he sustained an injury to his left knee. He required surgery for this injury, which was performed on August 27, 2010.  He was able to return to work on November 29, 2010.

 

Mr. Negoda was paid a total of $19,834.41, which represents 17.4 weeks of lost time benefits for the injury that occurred on July 20, 2010.

 

On March 16, 2011, he sustained injuries to his left hand, left ring and small fingers. The injuries were not severe, but he did lose time off from work. He initially returned to work on April 18, 2011.

 

Mr. Negoda was paid a total of $5,280.32, which represents 4.6 weeks of lost time benefits for the injury that occurred on March 16, 2011.

 

On September 22, 2011, he sustained an injury to his right arm. This injury only required conservative treatment. There was no lost time involved.

 

On September 25, 2013, he sustained injuries to his lower back and right arm. The injuries required conservative care. There was no lost time involved. 

 

Mr. Negoda filed multiple Applications of Adjustment of Claims with the IWCC through his counsel. Subject to the approval of the IWCC, these cases can now be settled for $40,035.87, representing 21% loss of a left leg and 5% loss of a right arm, plus $1,000.00 as final resolution of disputed benefits.  This settlement will close out any future lost time or medical benefits associated with these injuries.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
