Authority to settle the Workers’ Compensation Claim of Roger Pray vs. MWRDGC, Case 15 WC 28699, Illinois Workers’ Compensation Commission (IWCC), in the sum of $27,631.53, Account 901-30000-601090

Dear Sir:

 

Roger Pray is an Operating Engineer I at the Stickney Water Reclamation Plant.  On February 6, 2015, he slipped and sustained an injury to his right ankle. Mr. Pray was initially seen at the Industrial clinic on February 6, 2015.  Mr. Pray was released to restricted duty on July 1, 2015. He was able to resume his regular duties on July 20, 2015. 

 

Mr. Pray was paid a total of $24,728.92, which represents 20.8 weeks of lost time benefits.

 

Mr. Pray’s attorney filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC).  Subject to the approval of the IWCC, this case can now be settled for $27,631.53, representing 22.5% loss of right foot. This settlement will close out future lost time benefits, as well as any future medical benefits, associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
