Dear Sir:
Request for Proposal documents are being prepared for a Dual-Choice Dental Plan Administrator at the request of the Human Resources Department.
The purpose of this contract is to select an administrator for the District’s PPO and DMO dental insurance plans for employees. The services are currently provided by Blue Cross Blue Shield of Illinois, a division of Health Care Services Corporation.
No bid deposit is required for this request for proposal.
The Affirmative Action Ordinance Appendix A will be included in this contract. The type of work to be performed under the contract is within the professional services category for establishing Minority-owned Business Enterprises (MBE), Women-owned Business Enterprises (WBE) and/or Small Business Enterprises (SBE) utilization goals. The MBE, WBE and/or SBE utilization goals for this contract are: 20% percent MBE, 10% percent WBE, and 10% percent SBE.

Funds for years 2017, 2018 and 2019 are contingent on the Board of Commissioners’ approval of the District’s budget for those years.
