Authority to settle the Illinois Workers’ Compensation Claim of John Radavich vs. MWRDGC, Claim W000838040, in the sum of $16,000.00, Account 901-30000-601090

Dear Sir:
John Radavich is a Machinist at the Stickney Water Reclamation Plant. On August 2, 2012, the employee sustained an injury to his left wrist. He was initially seen at the Industrial clinic on August 2, 2012. He was treated conservatively with medicines and splinting, but his condition did not improve. The employee underwent a procedure repair on May 5, 2014. He was disabled effective May 7, 2014. He was able to return to work on May 22, 2014.
Mr. Radavich was paid a total of $2,559.37 in lost time benefits, which represents approximately 2.2 weeks of lost time benefits for the injury that occurred on August 2, 2012.
Subject to the approval of the Illinois Workers’ Compensation Commission (IWCC) this Pro-Se case can now be settled for $16,000.00, representing approximately 11.82% loss of the left hand. This settlement will close out future lost time benefits, as well as any future medical benefits associated with this injury.
The Acting Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
