Dear Sir:

 

Curtis Mason is a retired Maintenance Laborer A, who worked at the Stickney Water Reclamation Plant.  On January 13, 2015, he sustained an injury to his left shoulder.  He was initially seen at the Industrial clinic on January 13, 2015.  He was treated conservatively with medicines and therapy, but his condition did not improve.  The employee underwent a procedure on June 22, 2015.  He was disabled effective June 22, 2015.  He was able to return to work on August 13, 2015.

 

Mr. Mason was paid a total of $7,343.61 in lost time benefits, which represents approximately 7.6 weeks of lost time benefits for the injury that occurred on January 13, 2015.

 

Subject to the approval of the Illinois Workers’ Compensation Commission (IWCC) this Pro-Se case can now be settled for $40,455.35, representing approximately 21.7% loss of the left arm.  This settlement will close out any future medical benefits associated with this injury.

 

The Acting Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
