Dear Sir:

 

Michael Roman is an Ironworker at the Calumet Water Reclamation Plant.  On November 7, 2013, the employee sustained an injury to his left shoulder.  He was initially seen at the Industrial clinic on November 7, 2013.  The employee was disabled effective January 22, 2014.  He underwent a procedure on February 10, 2014.  Following the procedure he underwent a course of physical therapy.  He was able to return to work on August 22, 2014. 

 

On September 24, 2014, he sustained an injury to his right shoulder at the Calumet Water Reclamation Plant.  He was initially seen at the Industrial clinic on September 24, 2014.  Mr. Roman was disabled after undergoing a procedure on December 15, 2014.  He returned to work initially on January 21, 2015, but was disabled again effective January 22, 2015.  His treatment following the procedure consisted of physical therapy.  He was able to return to work on June 24, 2015.

 

Mr. Roman was paid a total of $27,768.32 in lost time benefits, which represents 25.2 weeks of lost time benefits for the injury that occurred on November 7, 2013.

 

Mr. Roman was paid a total of $30,730.08 in lost time benefits, which represents 27.2 weeks of lost time benefits for the injury that occurred on September 24, 2014.

 

Mr. Roman filed multiple Applications of Adjustment of Claims with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, these cases can now be settled for $90,000.00, representing approximately 24.942% loss of a person as a whole. This settlement will close out any future lost time or medical benefits associated with these injuries.

 

The Acting Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
