Authority to settle the Illinois Workers’ Compensation Claims of Marc Hayes vs. MWRDGC, Claim number: W001117439 in the sum of $27,062.25, Account 901-30000-601090

Dear Sir:
Marc Hayes is a Truck Driver at the Stickney Water Reclamation Plant. On February 3, 2014, he sustained an injury to his right shoulder. He was initially seen at the industrial clinic on February 3, 2014. He was treated conservatively with physical therapy and injections. Surgical intervention was recommended after Mr. Hayes underwent further testing. Mr. Hayes chose not to undergo surgery.
Mr. Hayes did not lose any time from work as a result of his accident.
Subject to the approval of the IWCC, this Pro-Se case can be settled for $27,062.25, representing 7.5% loss of a person as a whole. This settlement will close out any future lost time benefits as well as any future medical benefits associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
