Dear Sir:

 

Sheryl Gorski is a Laborer Foreman at the Stickney Water Reclamation Plant.  On April 29, 2015, she sustained injuries to her neck, back, head and left arm. Ms. Gorski was initially seen in the hospital emergency room on April 29, 2015. She was disabled from work effective April 30, 2015. She underwent a conservative treatment for her multiple injuries consisting of physical therapy and medication. She was able to return to work on July 30, 2015.  On January 27, 2016 she was disabled as she underwent surgery to repair her left shoulder.  Following this surgery the employee underwent additional physical therapy.  Ms. Gorski was able to return to work on March 21, 2016.

 

Ms. Gorski was paid a total of $21,415.76 representing 20.60 weeks of lost time benefits.

 

Ms. Gorski filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through her counsel. Subject to the approval of the IWCC, this case can now be settled for $65,870.76, representing approximately 4% loss of use of the person as a whole for the neck, back and head and 13.915% loss of the  person as a whole for her left arm. This settlement will close out future lost time benefits, as well as any future medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
