Dear Sir:

 

Margaret Hruby is a Police Officer at the Calumet Water Reclamation Plant.  On July 6, 2013, she sustained an injury to her neck.  Ms. Hruby was initially seen by the doctor at the industrial medical clinic on July 6, 2013.  The employee was disabled from work initially on July 11, 2013.  Her treatment consisted of physical therapy, traction and a home exercise program.  She returned to work on August 5, 2013, but was disabled again effective August 13, 2013.  She was able to resume work effective December 23, 2013.

 

Ms. Hruby was injured again on May 20, 2016 when she sustained a new injury to her neck.  The employee received conservative treatment for this injury consisting of physical therapy. She did not lose any time from work for this injury.

 

Ms. Hruby received Police Disability benefits for the time that she missed work. 

 

Ms. Hruby filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through her counsel.  Subject to the approval of the IWCC, these cases can now be settled for $28,866.40, representing approximately 8% loss of person as a whole.  This settlement will close out future lost time benefits, as well as any future medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
