Authority to settle the Workers’ Compensation Claim of David Robertson vs. MWRDGC, Claim 12 WC 17093, Illinois Workers’ Compensation Commission (IWCC), in the sum of $14,000.00, Account 901-30000-601090

Dear Sir:
David Robertson is a Carpenter at the Stickney Water Reclamation Plant. On September 2, 2011, he sustained injuries to his eye, nose, lip, mouth and hand. Mr. Robertson was initially seen in the industrial clinic on September 2, 2011. He underwent conservative treatment for his injuries, but did have to see a Dentist as he required extensive dental work for broken teeth. Mr. Robertson did not lose any time off work for his injury.
Mr. Robertson filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, this case can now be settled for $14,000.00, representing approximately 4% loss of person as a whole. This settlement will close out future lost time benefits, as well as any future medical benefits associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
