Dear Sir:

 

Joseph Francis is a Plumber at the Calumet Water Reclamation Plant.  On November 12, 2014, he sustained injuries to his left knee. Mr. Francis was initially seen in the industrial clinic on November 12, 2014. He was disabled from work effective November 14, 2014.  He initially underwent conservative treatment consisting of physical therapy and medication.  On February 2, 2015 he underwent a total left knee replacement.  Following this surgery the employee underwent additional physical therapy.  Mr. Francis was able to return to work on July 28, 2015. 

 

Mr. Francis was paid a total of $44,247.13 representing 36.50 weeks of lost time benefits.

 

Mr. Francis filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel.  Subject to the approval of the IWCC, this case can now be settled for $63,241.82, representing approximately 40% loss of use of the left leg.  This settlement will close out future lost time benefits, as well as any future medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
