Dear Sir:

 

Zenobia Williams is a Treatment Plant Operator I at the Calumet Water Reclamation Plant. On January 9, 2014, she sustained injuries to her right ankle. Ms. Williams was initially seen in the industrial clinic on January 9, 2014. She was disabled from work initially on February 1, 2014 and was able to return to work on April 14, 2014. Her treatment was conservative in nature consisting of physical therapy and medications.

 

Ms. Williams was paid a total of $ 8,187.31 in lost time benefits for a total period of 10.4 weeks.

 

Ms. Williams filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through her counsel. Subject to the approval of the IWCC, this case can now be settled for $11,832.11, representing approximately 10% loss of a right foot. This settlement will close out future lost time benefits, as well as any future medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
