Authority to settle the Illinois Workers’ Compensation Claim of Johnson Olowu vs. MWRDGC, Claim Number: W001690520 in the sum of $43,146.51, Account 901-30000-601090

Dear Sir:

 

Johnson Olowu was a Computer Systems Administrator at the Main Office Building.  He has since retired from the District effective January 18, 2018.  On May 26, 2016, he sustained an injury to his right shoulder.  He was initially seen by his personal physician on May 26, 2016.  The employee initially treated conservatively with physical therapy and medications.  He failed to improve and was disabled from work effective October 13, 2016.  On October 16, 2016 he underwent a surgical procedure on his right shoulder.  Following this procedure he underwent additional physical therapy.  He returned to work on January 3, 2017. 

 

Mr. Olowu was paid a total of $16,219.50 in lost time benefits which represents a total of 11.6 weeks of lost time benefits. 

 

Subject to the approval of the IWCC, this Pro-Se case can be settled for $43,146.51 representing approximately 11.132% person as a whole.  This settlement will close out any future lost time benefits as well as any future medical benefits associated with these injuries.   

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
