Dear Sir:

 

In 2014, Rashied Muhammad was a Maintenance Laborer B employee at the Calumet Water Reclamation Plant.  On November 12, 2014, he sustained injuries to his right hand and wrist. 

Mr. Muhammad was initially seen in the industrial clinic on November 12, 2014.  He was initially treated conservatively with medications and restrictions.  He was disabled effective June 22, 2015 after he underwent a procedure on his right wrist and hand.  Mr. Muhammad was able to return to work on November 2, 2015.   

 

Mr. Muhammad was paid a total of $12,148.22 representing 19 weeks of lost time benefits.

 

Mr. Muhammad filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel.  Subject to the approval of the IWCC, this case can now be settled for $14,745.65, representing approximately 12.5% loss of use of the right hand.  This settlement will close out future lost time benefits, as well as any future medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
