Authority to settle the Workers’ Compensation Claim of Michael Chrostek vs. MWRDGC, Claim number 14 WC 9732, Illinois Workers’ Compensation Commission (IWCC), in the sum of $23,273.53, Account 901-30000-601090

Dear Sir:

 

Michael Chrostek, currently a Senior Civil Engineer, was an Associate Structural Engineer at the Stickney Water Reclamation Plant at the time of his injury. On January 22, 2014, he sustained injuries to his right leg after slipping on some ice while at Stickney Water Reclamation Plant. Mr. Chrostek was initially seen in industrial clinic on January 22, 2014. He was initially disabled from work effective January 23, 2014. He returned to work on January 27, 2014. His initial medical treatment was conservative in nature. The employee was disabled again effective April 8, 2014 after undergoing a surgical procedure of his right leg. Following the surgery Mr. Chrostek underwent a course of physical therapy. He returned to work on June 4, 2014.     

 

The employee received lost time benefits during his disability totaling $10,547.90 representing a total of 8.60 weeks paid.  

 

Mr. Chrostek filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $23,273.53, representing approximately 15% loss of use of the right leg. This settlement will close out future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
