Dear Sir:
Request for Proposal documents are being prepared for a health plan administrator for a three-year period at the request of the Human Resources Department.
The purpose of this contract is to select a qualified carrier to provide medical and prescription drug coverage for employees and non-Medicare eligible retirees. The program will include: a preferred provider organization (PPO), a health maintenance organization (HMO) and prescription drug coverage. The services are currently provided by Blue Cross Blue Shield of Illinois, a division of Health Care Services Corporation. The current contract expires December 31, 2019.
No bid deposit is required for this request for proposal.
The Diversity Section reviewed the Request for Proposal and determined that Appendix A should be included in this contract. The Affirmative Action goals for this Request for Proposal are 15% Minority Business Enterprises (MBE) and/or Women Business Enterprises (WBE) and 10% Small Business Enterprises (SBE).
The tentative schedule for this contract is as follows:

Funds for years 2020, 2021 and 2022 are contingent on the Board of Commissioners’ approval of the District’s budget for those years.
