Report on the Settlement of Workers’ Compensation Claims and Miscellaneous Claims under $10,000.00

Dear Sir:

 

The following matters were settled upon the recommendation of the Director of Human Resources and the approval of the Executive Director:

 

1.      Settlement of Workers’ Compensation Claim of Jason Mayans, Claim No. W002000696, in the amount of $3,416.60.

 

Mr. Mayans, a Sheet Metal Worker, alleges a May 4, 2017 work-related injury when he drilled into his left hand on the dorsal side. Mr. Mayans sought medical treatment but later developed cellulitis and was admitted into the hospital.  His condition improved after intense IV antibiotics.  The settlement amount represents approximately 2.15% loss of use of the left hand.

 

2.      Settlement of Workers’ Compensation Claim of Eliezer Medina Jr., Claim No. 16WC14667, in the amount of $750.00.

 

Mr. Medina, a Maintenance Laborer A, alleges an April 29, 2016 work-related injury when he was poked in the lower extremity by a fish bone sticking out of a bag while removing dead fish from a District truck. Mr. Medina sought medical treatment but later developed cellulitis and was admitted into the hospital.  IV antibiotics were administered.  The settlement amount represents approximately 0.462% loss of use of the left leg.

 

3.     Settlement of Workers’ Compensation Claim of Larry Starks, Claim No. 18WC12696, in the amount of $3,953.20.

 

Mr. Starks, a Treatment Plant Operator II, alleges an April 4, 2018 work-related injury when he lifted and dragged a heavy rope for 60 feet. After performing this task, he felt severe lower back pain, right leg pain and groin pain.  He was initially seen at the emergency room.  He returned to work but continued to have pain.  He was placed on restricted duty and his treatment consisted of extensive physical therapy and medications.  The settlement amount represents approximately 1% loss of use of the person as a whole.

 

4.      Settlement of Automobile Damage Claim of Stephen Bach, Claim No. A/P 1470, in the amount of $777.80.

 

On May 18, 2019, a vehicle driven by Mr. Bach was rear-ended by a District employee driving a District vehicle. The settlement amount is for vehicle repairs.
