Dear Sir:
 
Darryl Carr is a Maintenance Laborer A Shift at the Calumet Water Reclamation Plant.  On January 22, 2010, he injured his left knee at work.  On February 22, 2010, an MRI of the left leg revealed a mild increased signal in the posterior horn of the medial meniscus, favorable for mucoid degeneration.  Mr. Carr received prescribed medications and physical therapy for this injury.
 
Mr. Carr was paid a total of six weeks of lost time benefits for this claim.
 
On March 18, 2010, Mr. Carr sustained an injury to his left leg and mid/low back.  On April 12, 2010, an MRI of the lower back revealed lumbar disc displacement at L5-S1 with right lateral recess narrowing, right facet arthrosis/hypertrophy at L5-S1, lumbar radiculitis, and sciatica.  He received a series of epidural injections for this injury.
 
Mr. Carr was paid a total of eight weeks of lost time benefits for this second claim.
 
Mr. Carr filed two Applications for Adjustment of Claim with the IWCC through his counsel.  Subject to approval of the IWCC, these claims can now be settled for the sum of $21,060.18, which represents 5% loss of use of the body as a whole ($16,741.00) for the January 22, 2010, injury and 3% loss of use of the left leg ($4,319.18) for the March 18, 2010, injury.  This settlement will close out any future lost time benefits as well as any future medical exposure on these claims.
 
The General Counsel believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
