Dear Sir:
 
The North Palos Fire Protection District ("NPFPD") requested a Permit for Confined Space Entry at the District's Palos Hills Pumping Station at 10700 S. 76th Avenue in Worth, Cook County, Illinois, for confined space entry training including air quality monitoring, hazard assessment, ventilation, access safety, patient rescue and care.  A map depicting the District facilities at the subject location is attached.
 
The District's technical departments have reviewed NPFPD's request for a one (1) month Permit for Confined Space Entry and have no objections thereto, subject to the permittee ensuring that the District's facility will not be compromised.  NPFPD should also comply with State and Federal OSHA regulations, as well as with the District's "Confined Space Permit" and "Confined Space Entry Procedures".
 
A nominal fee of $10.00 is recommended as is customary for governmental entities using District land for public purposes.  It is also in the District's best interest to allow the training so that NPFPD can be familiar with this facility in the event of an emergency.
 
It is requested that the Executive Director recommend to the Board of Commissioners that it authorize and approve the issuance of a one (1) month Confined Space Entry Permit to the North Palos Fire Protection District to conduct training activities at the District's Palos Hills Pumping Station at 10700 S. 76th Avenue in Worth, Cook County, Illinois. Consideration shall be a nominal fee of $10.00.
 
It is also requested that the Executive Director recommend to the Board of Commissioners that it authorize and direct the Chairman of the Committee on Finance and the Clerk to execute said Confined Space Entry Permit on behalf of the District after it is approved by the General Counsel as to form and legality.
