Dear Sir:
 
Anthony Blejski is an Electrical Mechanic who works at the Stickney Water Reclamation Plant.  On January 10, 2011, he injured his lower back at work.  
 
On February 1, 2011, he was seen at the industrial clinic where an MRI of the lumbar spine was performed revealing an L4-L5 level disc desiccation and moderate decreased disc height, circumferential disc bulge, asymmetric to the left with broad based central protrusion causing moderate spinal stenosis.  At the L3-L4 level, there was a mild circumferential disc bulge.  
 
On March 14, 2011, Mr. Blejski was given an epidural steroid injection.  On April 4, 2011, he received a second epidural steroid injection.
 
A total of five weeks of lost time benefits were paid for this injury.
 
Mr. Blejski filed an Application for Adjustment of Claim with the IWCC through his counsel.  Subject to approval of the IWCC, this claim can now be settled for the sum of $25,111.50, which represents 7.5% loss of use of the body as a whole.  This settlement will close out any future lost time benefits as well as any future medical exposure on this claim.
 
The General Counsel believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
