Dear Sir:
 
On April 16, 2009, the Board of Commissioners authorized the Director of Procurement and Materials Management to award Contract 09-640-11, Furnishing and Delivering Sodium Bisulfite to Various Locations in the North Service Area, to PVS Chemical Solutions, Inc., in an amount not to exceed $156,400.00. The contract expired on December 31, 2012.
 
As of December 18, 2012, the attached list of change orders has been approved. The effect of these change orders resulted in an increase in an amount of $126,380.00 from the original amount awarded of $156,400.00. The current contract value is $282,780.00. The prior approved change orders reflect an 80.8% increase to the original contract value.
 
A decrease in the contract value is being requested because the actual expenditure was less than originally anticipated, and to close-out the Contract.
 
This change order is in compliance with the Illinois Criminal Code since the change is due to circumstances not reasonably foreseeable at the time the contract was signed, and is in the best interest of the District.
 
It is hereby recommended that the Board of Commissioners authorize the Director of Procurement and Materials Management to execute a change order to decrease Contract 09-640-11, in an amount of $18,212.27 (approximately 6.4% of the current contract value) from an amount of $282,780.00 to an amount not to exceed $264,567.73.
 
Funds will be restored to Account 101-67000-623560.
