Dear Sir:
 
Michael Eberhart is an Electrician at the Stickney Water Reclamation Plant.  On June 6, 2011, he injured his right knee and right shoulder at work.
 
Mr. Eberhart was diagnosed with a meniscus tear in his right knee, and a torn bicep and multiple tears in his right shoulder rotator cuff.
 
On August 5, 2011, he underwent right knee arthroscopic surgery.  No physical therapy was necessary post surgery as rehabilitation was limited to home exercise.  On August 30, 2011, Mr. Eberhart underwent right shoulder arthroscopic surgery.  Extensive physical therapy was required post surgery.
 
Mr. Eberhart was paid a total of 42 weeks of lost time benefits for this injury.
 
Mr. Eberhart filed an Application for Adjustment of Claim with the IWCC through his counsel.  Subject to approval of the IWCC, this claim can now be settled for the sum of $80,331.42, which represents 17.7% loss of use of the body as a whole and 15% loss of use of the right leg.  This settlement would also close out any future medical exposure on this claim.
 
The General Counsel believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
