Authority to settle the Workers' Compensation Claim of Andrew O'Neill vs. MWRDGC, Claim No. 0200005067, Illinois Workers' Compensation Commission (IWCC), in the sum of $25,111.50. Account 901-30000-601090

Dear Sir:
Andrew O'Neill is a Pipe Coverer who works at the Stickney Water Reclamation Plant. On February 4, 2011, he injured his lower back at work.
On February 7, 2011, Mr. O'Neill was diagnosed with back pain. On February 10, 2011, an MRI of the lumbar spine was performed revealing multilevel degenerative disease as well as fairly significant protrusion at the L5-S1 level that appears to be effacing the left S1 nerve root. His treatment consisted of two epidural steroid injections and physical therapy.
A total of four weeks of lost time benefits was paid for this injury.
Subject to approval of the IWCC, this claim can now be settled for the sum of $25,111.50 representing 7.5% loss of use of the body as a whole. This settlement will close out any future lost time benefits as well as any future medical exposure on this claim.
The General Counsel believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
