Dear Sir:

 

Keith Conry is a Pipefitter at the Calumet Water Reclamation Plant.  On October 19, 2007, while removing a welded spool piece from the welding bench to a cart, he strained his right shoulder.  He saw his personal doctor who referred him for an MRI which showed a rotator cuff tear and impingement.  Mr. Conry underwent right shoulder arthroscopy on December 24, 2008, in which the doctors performed a debridement of undersurface rotator cuff tear, a biceps tendonesis, repair of partial rotator cuff tear, and a subacromial decompression.  He then completed a physical therapy program and was cleared to return to work without restrictions.  Mr. Conry was off work for 25 weeks.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this pro se claim can be settled for the sum of $36,209.65 which represents a 22.5% loss of use of the right arm.

 

The General Counsel believes this is an equitable settlement to conclude this matter and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
