Dear Sir:

 

Diane Heirman is a Maintenance Laborer A at the Stickney Water Reclamation Plant.  On October 1, 2008, she reported right elbow pain after lifting sample carboys throughout the day.  She went to Excel Occupational Health and was diagnosed with right lateral epicondylitis.  Conservative treatment including therapy and a steroid injection provided only temporary relief of her pain.  On March 18, 2009, she underwent surgical repair of the lateral epicondyle.  Heirman completed a physical therapy program and returned to regular duties on July 1, 2009.  She lost 173 days of work.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this pro-se claim can be settled for the sum of $25,226.12 which represents a 15% loss of use of the right arm.

 

The General Counsel believes this is an equitable settlement to conclude this matter and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
