Dear Sir:

 

Angela Anderson is a Fireman Oiler at the Hanover Park Water Reclamation Plant.  On October 4, 2007, she was moving a heavy metal cover to check oil and drain condensate; the plate slipped, and when she tried to catch it, she strained her neck and left shoulder.  Left shoulder MRI on December 28, 2007, revealed a rotator cuff tear.  Ms. Anderson underwent surgical repair on March 11, 2008.

 

While undergoing post surgery physical therapy, Ms. Anderson developed numbness, tingling, and weakness in her left and right hands.  EMGs revealed bilateral carpal tunnel syndrome (CTS).  The treating physician opined that repetitive duties at work over the years was a contributing factor to the CTS diagnosis.  Ms. Anderson underwent left CTS release on March 10, 2009, and right CTS release on April 28, 2009.  After additional physical therapy, she returned to regular duties on August 7, 2009.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this pro-se claim can be settled for the sum of $79,359.71 which represents a 25% loss of use of the left arm ($40,236.49), 15% loss of the right hand/wrist ($19,561.61), and 15% loss of the left hand/wrist ($19,561.61).

 

The General Counsel believes this is an equitable settlement to conclude this matter and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
