Dear Sir:

 

Ayanna Smith is a Maintenance Laborer A at the Stickney Water Reclamation Plant.  On December 18, 2007, while taking garbage out of the truck garage, she slipped and fell on ice outside the overhead door.  She was taken to LaGrange Memorial Hospital and diagnosed with a trimalleolar fracture of the left ankle.  She underwent surgery the next day that included open reduction and internal fixation.  Ms. Smith completed a physical therapy program and returned to work on June 3, 2009.  After her return to work, Ms. Smith had trouble with swelling caused by the hardware in her ankle.  She underwent an injection and additional therapy.  She lost a total of 134 days of work.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this pro se claim can be settled for the sum of $36,383.29 which represents a 40% loss of the left foot/ankle.

 

The General Counsel believes this is an equitable settlement to conclude this matter and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
