Dear Sir:

 

Authorization is requested to issue a purchase order and enter into an agreement with Accelerated Rehabilitation Centers, Ltd. for On-Site Physical Therapy. 

 

The terms of the request for proposal require the firm to provide physical therapy services (on a one-on-one basis) to employees who have had injuries on the job and have been given a prescription for physical therapy by their doctors.

 

The selected firm is required to (a) provide all the traditional physical therapy modalities; (b) emphasize the use of the work environment for rehabilitation using job-related tasks to accomplish this goal; (c) keep a schedule of visits and provide ongoing evaluations to District representatives; (d) provide all necessary equipment; and (e) provide registered physical therapists.  Upon approval, the selected firm will provide physical therapy services on-site at the Stickney Plant.

 

The contract term for the On-Site Physical Therapy Program will begin upon the approval of the contract by the Board of Commissioners and execution of an agreement that will continue until December 31, 2011, with the discretion to extend the contract for an additional one-year period utilizing the same fee schedule.

 

Request for Proposal 10-RFP-01 was advertised on February 10, 2010.  Forty-nine (49) firms were notified, and seven (7) firms requested proposal documents.  The District received proposals from three (3) firms on March 12, 2010, as indicated below:

Accelerated Rehabilitation Centers, Ltd.

Ingalls Memorial Hospital

 

The proposals were evaluated in accordance with the criteria outlined in the Request for Proposal 10-RFP-01, encompassing all aspects including cost.  Firms submitting bids were interviewed by a panel of District representatives.  Accelerated Rehabilitation Centers, Ltd. (“Accelerated”) was selected as the firm best suited to provide the required professional services.

 

Accelerated has a network of clinic locations throughout the Chicagoland area.  Their physical therapists on staff have extensive experience to perform this service.  The cost proposal submitted by Accelerated was considered a fair and reasonable price.  We anticipate that a cost savings to the District will continue to occur by offering this service at the Stickney Plant.  That cost savings is reflected in the reduced charges we pay and less time lost from work by District employees.

 

The Affirmative Action Section recommends that Appendix A not be included because the service is not divisible and will be performed by only one physical therapist.

 

Inasmuch as Accelerated posses a high degree of professional skill, it is recommended that the Director of Procurement and Materials Management be authorized to issue a purchase order to enter into an agreement per Section 11.4 of the Purchasing Act with Accelerated Rehabilitation Centers, Ltd. in an amount not to exceed $200,000.00.

 

Funds are available in Account 901-30000-601090.
