Dear Sir:

 

On April 3, 2006, Donald Dye was a Maintenance Laborer B at the North Side Water Reclamation Plant.  On the morning of April 3, he was helping clear a blockage from the Deerfield Reservoir area.  A co-worker working nearby fell into the Chicago River North Branch West Fork adjacent to the Deerfield Reservoir while clearing debris from a culvert and yelled for help.  Mr. Dye was one of the co-workers that assisted in an unsuccessful rescue attempt.

 

Mr. Dye received treatment for a diagnosis of Post-Traumatic Stress Disorder.  He lost 255 days of work.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, the claim can be settled for the sum of $11,027.36 which represents a 4.3% loss of the person as a whole and two weeks of disputed disability pay.

 

The General Counsel believes this is an equitable settlement to conclude this matter and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
