Dear Sir:
On January 18, 2007, the Board of Commissioners authorized the Director of Procurement and Materials Management to issue purchase order to FLC Instruments, Inc., to furnish and deliver parts on an as-needed basis, for Scott Digital Multi-Channel Gas/Vapor Systems to the North Side Water Reclamation Plant, in an amount not to exceed $40,000.00. The contract was to expire on December 31, 2011; however, FLC Instrument, Inc., was dissolved prior to the contract expiration date.
This contract has no prior change orders.
A decrease is being requested at this time because of the involuntarily dissolution of FLC Instruments, Inc.
This change order is in compliance with the Illinois Criminal Code since the change is due to circumstances not reasonably foreseeable at the time the contract was signed, and is in the best interest of the District.
It is hereby recommended that the Board of Commissioners authorize the Director of Procurement and Materials Management to execute a change order to decrease the purchase order in an amount of $37,083.00 (92.71% of the current total contract value), from an amount of $40,000.00, to an amount not to exceed $2,917.00.
Funds will be restored in Account 101-67000-623780.
