Authority to settle the Workers’ Compensation Claim of Andrea Mucino vs. MWRDGC, Case No. 09 WC 11185, Illinois Workers’ Compensation Commission, in the sum of $13,595.36, Account 901-30000-601090

Dear Sir:

 

Andrea Mucino is a Pollution Control Technician I at the North Side Water Reclamation Plant.  On October 20, 2008, she lifted an auto sampler battery, turned to place it into a vehicle, and felt pain in her upper back.  She went to Peterson Urgent Care Center and was diagnosed with mid-thoracic and right shoulder strains.  Ms. Mucino underwent an MRI scan on November 13, 2008, which revealed a partial rotator cuff tear and subacromial impingement.  She completed a physical therapy program and returned to work on December 8, 2008.  She lost seven weeks of work.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this claim can be settled for the sum of $13,595.36 which represents a 10% loss of use of the right shoulder.

 

The General Counsel believes this is an equitable settlement to conclude this matter and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
