Oakton Street, in Des Plaines, Illinois, to read, inspect and maintain its City-owned water meters, consideration shall be a nominal fee of $10.00

Dear Sir:

 

On June 15, 1995, February 3, 2000 and May 19, 2005, respectively, the Board of Commissioners approved a 5-year confined space entry permit to the City of Des Plaines (“City”) for its Department of Public Works to access meter vaults at the James C. Kirie Water Reclamation Plant, located at 701 W. Oakton Street, in Des Plaines, Illinois.  Periodic access by City staff to these vaults is necessary to read, inspect and maintain City-owned water meters that monitors water usage at the James C. Kirie Water Reclamation Plant.

 

The current permit has expired and the City has requested a new 5-year permit to continue to periodically access confined space meter vaults at the subject location to read, inspect and maintain its water meters.

 

The City reports that its staff is fully trained and equipped to perform this work while complying with all OSHA/IDOL requirements for confined space entry.  The City of Des Plaines will also be required to certify compliance with OSHA/IDOL requirements.  The District’s technical departments have reviewed the request and have no objections thereto.

 

As in the past, the City has requested a waiver of all fees, a waiver of the five thousand dollars ($5,000.00) indemnity bond requirement and a waiver of environmental contamination insurance coverage.  There are no objections to these requests.  The permit will be subject to the same terms and conditions as previous permits including a nominal consideration of $10.00, the procurement of $4,000,000.00 general liability, property damage and worker’s compensation insurance coverage.

 

It is requested that the Executive Director recommend that the Board of Commissioners authorize and approve the issuance of a 5-year permit to the City of Des Plaines to periodically access confined space meter vaults located at the James C. Kirie Water Reclamation Plant, located at 701 W. Oakton Street, in Des Plaines, Illinois, to read, inspect and maintain its City-owned water meters.  Consideration shall be a nominal fee of $10.00.

 

It is further requested that the Executive Director recommend to the Board of Commissioners that the Chairman of the Committee on Finance and the Clerk be authorized and directed to execute said confined space entry permit agreement after same is approved by the General Counsel as to form and legality.
