Dear Sir:

 

Joseph Ambroz was a Plumber at the Stickney Water Reclamation Plant.  He retired on September 1, 2008.  On May 3, 2007, he complained of bilateral shoulder pain while pulling on pipe wrenches to install a four-inch galvanized pipe overhead.  Initial assessment at the Clearing Clinic was bilateral shoulder strains.  Physical therapy and work restrictions were recommended.  On May 31, 2007, MRIs revealed a full thickness rotator cuff tear on the right shoulder and a partial tear on the left shoulder.

 

On July 10, 2007, Mr. Ambroz underwent right rotator cuff repair.  He completed a physical therapy program.  No additional treatment was undertaken for the left shoulder.  Mr. Ambroz lost 161 days of work.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this claim can be settled for the sum of $54,898.34 which represents a 27.5% loss of use of the right arm and a 7.5% loss of use of the left arm.

 

The Acting General Counsel believes this is an equitable settlement to conclude this matter and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
