Authority to settle the Workers’ Compensation Claim of Ken Kocian vs. MWRDGC, File No. 200003335, Illinois Workers’ Compensation Commission, in the sum of $23,372.87.  Account 901-30000-601090

Dear Sir:

 

Ken Kocian is a Pipefitter at the Stickney Water Reclamation Plant.  On January 10, 2007, while unloading a pallet, a heavy valve rolled off the skid onto Mr. Kocian’s left foot.  He went to a Clearing Clinic and was diagnosed with a left foot contusion.  EMG studies revealed damage to the peroneal nerve.  Treatment included a solid brace, medication, and nerve stimulation therapy.

 

During treatment for his left foot, Mr. Kocian began having complaints of pain in his left hip and was diagnosed with trochanteric bursitis/left periformis syndrome.  The treating specialist reported that the left hip condition was caused by Mr. Kocian’s gait due to his left foot injury.  Treatment included three steroid injections into the area.  Mr. Kocian lost a total of 33 days of work.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this claim may now be settled for the sum of $23,372.87 which represents a 20% loss of the left foot ($20,707.00) and a 2% loss of the left leg ($2,665.87).

 

The Acting General Counsel believes this is an equitable settlement to conclude this matter and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
