Dear Sir:

 

Glenn Pacyga is a Storekeeper at the Stickney Water Reclamation Plant.  On March 2, 2010, he stepped off a forklift, approximately 18 inches off the ground, and twisted his left knee.  He went to the Excel Occupational Health Clinic and was diagnosed with a left knee strain.  He was treated conservatively but had persisting symptoms and was sent for an MRI.  An MRI revealed a medical meniscus tear.  Mr. Pacyga underwent left knee medial meniscectomy on April 9, 2010.  He completed a physical therapy program and returned to regular work on May 24, 2010.  He lost 31 days of work.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, the pro se claim can be settled for the sum of $25,010.09 which represents a 17.5% loss of the left leg.

 

The Acting General Counsel believes this is an equitable settlement to conclude this matter and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
