Dear Sir:
Donald Caldwell is a Maintenance Laborer A at the Stickney Water Reclamation Plant who was cleaning out a creek on November 23, 2010. After lifting a heavy log, he felt a sudden sharp pain in his abdomen. He went to the industrial clinic where he was diagnosed with an abdominal incisional hernia. Mr. Caldwell was referred to a surgeon, and on December 2, 2010, he underwent a repair of incisional ventral hernia with mesh.
Following his surgery, Mr. Caldwell remained disabled until being released to work on March 7, 2011. He lost a total of 14-2/5 weeks from work as a result of this accident.
Subject to the approval of the Illinois Workers’ Compensation Commission, this claim can be settled for the sum of $16,618.00 which represents 5% loss of use of body as a whole. This settlement closes out all future medical exposure related to this claim.
The General Counsel believes this settlement is in the District’s best interests and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
