Authority to settle the Workers' Compensation Claim of Kevin Devereux vs. MWRDGC, Case No. 12 WC 16245, Illinois Workers' Compensation Commission (IWCC), in the sum of $33,675.75. Account 901-30000-601090

Dear Sir:
Kevin Devereux is a Maintenance Laborer A (Shift) who works at the Stickney Water Reclamation Plant. On November 2, 2011, he injured his left knee at work. On March 1, 2012, he underwent surgery consisting of reconstruction of medial patellofemoral ligament with semitendinosus allograft. He underwent physical therapy post surgery and was able to return to work on April 30, 2012. Mr. Devereux was paid a total of 12 weeks of lost time benefits for this injury.
Mr. Devereux filed an Application for Adjustment of Claim with the IWCC through his counsel. Subject to approval of the IWCC, this claim can now be settled for the sum of $33,675.75, which represents 22.50% loss of use of the left leg. This settlement will close out any future lost time benefits as well as any future medical exposure on this claim.
The General Counsel believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
