Dear Sir:
Bernard Ellis is a Maintenance Laborer A (Shift) who works at the Stickney Water Reclamation Plant. On April 23, 2012, he injured his left shoulder at work. An MRI of the left shoulder indicated that he sustained a mild non-displaced fracture of the humeral head. After undergoing conservative treatment, Mr. Ellis was able to return to work on June 7, 2012. He was paid a total of six weeks of lost time benefits for this injury.
Subject to approval of the IWCC, this pro se claim can now be settled for the sum of $13,219.82, which represents 3.8% loss of use of the body as a whole. This settlement will close out any future lost time benefits as well as any future medical exposure on this claim.
The General Counsel believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
