Dear Sir:
Daniel Gierut is a Pipefitter at the Stickney Water Reclamation Plant. On February 6, 2007, he slipped on the last rung of a ladder and banged his right heel on the floor. He went to Clearing Clinic and was diagnosed with traumatic plantar fasciitis of the right foot. Treatment included medication, physical therapy, one injection and custom orthotics. He had continued heel pain.
On September 19, 2007, Gierut underwent an extracorporeal shockwave treatment on his right heel. He lost a total of 8 weeks of work.
Subject to the approval of the Illinois Workers’ Compensation Commission, this pro se claim can be settled for the sum of $10,353.50, which represents a 10% loss of the right foot.
The Attorney requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
