Dear Sir:
The following matters were settled upon the recommendation of the Director of Human Resources and the approval of the Executive Director:
1. Settlement of Workers’ Compensation Claim of Kelly Antonson, Claim No. 15WC011679, in the amount of $9,213.81.
Ms. Antonson, Electrical Instrument and Testing Mechanic, alleged a January 20, 2015 work related injury to her right foot/ankle after stepping out of a manhole using a steel rung ladder. The settlement amount represents approximately 7.5% loss of use of the right foot.
2. Settlement of Automobile Damage Claim of Christopher Tonn, Claim No. A/P 1491, in the amount of $5,604.45.
On February 26, 2020, a District snow plow struck the back portion of Mr. Tonn’s vehicle at the O’Brien Plant. The settlement amount is for vehicle repairs.
3. Settlement of Automobile Damage Claim of Stericycle, Inc., Claim No. A/P 1451, in the amount of $1,475.79.
On September 24, 20118, a District vehicle struck a vehicle leased by Stericycle, Inc The settlement amount is for vehicle repairs.
