Title
Authority to issue a 1-year permit to K-Five Construction Corporation, Inc. to access District Main Channel Parcels 29.01, 30.01, 30.03, 31.01, 31.02 and 31.03 between LaGrange Road and Willow Springs Road to repair areas of Interstate 294 known as the Mile Long Bridge in Willow Springs, Illinois. Consideration shall be a permit fee of $15,000.00.

Dear Sir:
K-Five Construction Corporation ("K-Five") has contracted with the Illinois State Toll Highway Authority ("ISTHA") to perform re-construction and repair of certain bridge structures along the northbound section of I-294 known as the "Mile Long Bridge." Mile Long Bridge is part of the I-294 Tollway System located between 95th Street and Cermak Road. K-Five has requested a 1-year permit to access District lands at or near LASMA to reach the bridge. The bridge is located within an ISTHA right-of-way. K-Five is requesting access to the bridge through three different points by traversing Main Channel Parcels 29.01, 30.01, 30.03, 31.01, 31.02 and 31.03.
The District's technical departments have reviewed the request and have no objections thereto. Main Channel Parcels 29.01, 30.01, 30.03, 31.01, 31.02, and 31.03 are leased to the Cook County Forest Preserve District. Any permit issued by the District will be conditioned upon the contractor obtaining the written consent of the Forest Preserve to traverse its leasehold.
Because the intended use is for daily road access over six parcels, a minimum $2,500.00 fee per parcel is recommended, or $15,000.00.
It is requested that the Executive Director recommend to the Board of Commissioners that it authorize and approve the issuance of a 1-year permit to K-Five Construction Corporation, Inc. to access District Main Channel Parcels 29.01, 30.01, 30.03, 31.01, 31.02 and 31.03 between LaGrange Road and Willow Springs Road to repair areas of Interstate 294 known as the Mile Long Bridge in Willow Springs, Illinois. Consideration shall be a permit fee of $15,000.00.
It is further requested that the Executive Director recommend to the Board of Commissioners that it authorize the Chairman of the Committee on Finance and the Clerk to execute said permit agreement after it is approved by the General Counsel as to form and legality.
