At the Board Meeting of October 18, 2012, the Board of Commissioners duly authorized the above stated action, Agenda Item No. 11, File No. 12-1422.
 
The requisition number has changed.
 
A textual error in the title/transmittal letter and resulting order indicated Requisition 1338962. Same should have read Requisition 1351587.
 
All other information provided in the transmittal is correct.
 
Therefore, it is requested that the aforesaid Board Order of October 18, 2012 be amended to effect the changes set forth above, otherwise to remain in force and effect as heretofore enacted.
