Authority to settle the Workers’ Compensation Claims of Ricardo Woods vs. MWRDGC, Claim numbers 12 WC 22215 and 15 WC 19694, Illinois Workers’ Compensation Commission (IWCC), in the sum of $24,999.00, Account 901-30000-601090

Dear Sir:

 

Ricardo Woods was a Painter at the Calumet Water Reclamation Plant.  He retired from the District on June 30, 2015.  On January 27, 2012, he sustained injuries to his lower back after he reported falling from scaffolding onto the ground. Mr. Woods was initially seen by the doctor at the industrial medical clinic on January 27, 2012.  The employee’s treatment consisted of physical therapy and medication.  He was disabled from work effective January 28, 2012.  He was able to return to work on May 15, 2012.   

 

Mr. Woods reported having a second accident on April 1, 2015 to his back when riding a three-wheel Cushman; he reported hitting a bump which caused an impact. He was initially seen by the doctor at the industrial medical clinic on April 1, 2015.  The employee’s treatment consisted of physical therapy and medication,

 

A total of $15,402.61 was paid in lost time benefits representing a total of 15.20 weeks.  

 

Mr. Woods filed an Application of Adjustment of Claim’s with the Illinois Workers’ Compensation Commission (IWCC) through his counsel.  Subject to the approval of the IWCC, these cases can now be settled for a total of $24,999.00, representing approximately 7.19% person as a whole for both claims.  This settlement will close out future lost time benefits associated with these injuries.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
