Dear Sir:
Dwayne Edwards was a Maintenance Laborer A Shift who retired, effective February 1, 2025. He was working at the Stickney Water Reclamation Plant on May 23, 2022, when he reported that while lifting sandbags out of a sewer he felt pain in his lower back. Mr. Edwards sought treatment at the Industrial Clinic on May 24, 2022, and was diagnosed with a back strain. He had an MRI on June 29, 2022, that revealed a bulging disc. He treated conservatively with physical therapy.
Mr. Edwards was not paid any lost time benefits.
Mr. Edwards filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, this case can be settled for $21,084.94 representing approximately 4.5% loss of use of the person as a whole. This settlement will close out any future lost time and medical benefits associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
